Should I fax, email, or mail forms to Safe at Home?
- Faxed or emailed copies of application materials and forms are not accepted.
- The advocate must sign and mail your application materials to Safe at Home.

Once enrolled, completed Safe at Home forms can be sent directly to Safe at Home via mail or fax. Some forms can be emailed to the email provided above, if you prefer. Please check with Safe at Home if you are uncertain if a Safe at Home form can be emailed.
